Second Story Housing is preparing to open as a small-household assisted living community in Champlin, Minnesota. We are working through the Minnesota Department of Health licensure process under Chapter 144G. This site shares our vision while the license is pending.

Second Story Housing is being built on the belief that every person deserves to live in a place that feels like home — a place where they are known, valued, and cared for. Once licensed, our home in Champlin, Minnesota will offer a small, family-style setting where residents and staff can build real relationships.
Second Story LLC was created to fill a real need in the Twin Cities metro area — supportive housing that doesn't feel institutional. Our planned home at 1013 West River Road in Champlin is being prepared so that, once licensed, residents will wake up in a safe, clean, welcoming environment and receive the personalized support they need to thrive.

We are committed to applying for and operating under a license issued by the Minnesota Department of Health under Chapter 144G, with ongoing staff training and continuous improvement once we open.
Our planned facility is located at 1013 West River Road in Champlin — a quiet residential neighborhood on the north metro. Champlin offers a peaceful, community-oriented setting with easy access to parks, shopping, and healthcare services.
Once licensed and open, we intend to welcome residents from across the Twin Cities metropolitan area, with an open-door approach to families as partners in care.

The descriptions below reflect the care model we are preparing to offer once Second Story is licensed by the Minnesota Department of Health under Chapter 144G. Specific service availability, scope, and pricing will be governed by the licensure category granted and the Uniform Disclosure of Assisted Living Services and Amenities (Minn. Stat. § 144G.83) at that time.
A safe, clean, furnished home environment in a residential neighborhood, with private living space inside a shared household. Planned upon licensure.
Home-cooked meals prepared with dietary needs and preferences in mind, including medical diets. Planned upon licensure.
Help with activities of daily living — bathing, grooming, dressing, mobility, hygiene — delivered with respect and discretion. Scope subject to licensure category.
Supervision and assistance with medication schedules, prescriber coordination, and documentation. Available only under the appropriate 144G licensure tier.
Daily activities, recreational opportunities, and social engagement to support mental health and community connection. Planned upon licensure.
Coordination support for medical appointments, community activities, and personal errands. Planned upon licensure.
Each future resident will receive a service plan developed collaboratively with the resident, family, and healthcare providers, in accordance with 144G.70. Planned upon licensure.
Coordination with residents' external healthcare teams to support continuity of care. Planned upon licensure.
Staffing model designed to meet 144G's 24-hour staff presence requirements. To be implemented once licensed and operational.
